Care Coordinators
Care Coordinators play an important role within a PCN (Primary Care Network) to proactively work with people, including the frail/elderly and those with long-term conditions, to provide coordination and navigation of care and support across health and care services. Care Coordinators will be based within our Single Point of Access (SPA) and the unique opportunity work across our services on a rotational basis. Our SPA covers the e-Hub, Secondary Care Referrals, Community Dermatology and Enhanced Cardiology Services.

The Current Process:

Practices will contact CLH with the required permanent or temporary role.

CLH will review the CLH bank staff database and select best suited staff with experience and availability in mind.

Staff details are then forwarded onto the requesting General Practice who will review candidates and to liaise with selected staff directly in regards of interviews, payments, hours of work and other information.

CLH cannot guarantee that a full - time position will be offered. However, staff bank opportunities that are fulfilled may lead to a permanent position and if so will be informed by GP practice.
